Receipts to T-105 restricted
Until the investigation into the 8 September high level alarm activation is complete, all receipts to hydrochloric acid tank T-105 must be supervised by the shift supervisor with a manual dip before and after and a maximum receipt of 20 tonnes. The ullage calculation must be checked by a second person and recorded on the transfer checklist. No exceptions.

Sampling protective equipment
Following the acid splash injury on 5 September, every person taking a sample from a process line must wear the chemical splash suit with the glove cuff over the sleeve, face shield and the respirator specified in the sample procedure. Supervisors will check protective equipment at the sample point during the first week. Report any sample point with a leaking valve or missing drain.

Safety case revision consultation
The five year revision of the Kwinana safety case is under way and the regulator has asked for evidence of worker consultation. Panel briefings will run over the next two roster cycles and the safety case summary will be available in the control room and the terminal office. Health and safety representatives will collect comments. Every operator is expected to attend one briefing.

October reactor shutdown
Reactor 2 will be shut down from 12 to 16 October for its annual internal inspection and relief valve testing. Confined space entry permits, the inspection plan and the contractor inductions are being prepared. Maintenance will hold the shutdown planning meeting on 25 September in the training room and all disciplines must send a representative.

Emergency exercise with emergency services
A full site emergency exercise involving emergency services and the neighbouring mutual aid sites will take place on 21 October. All personnel on site will take part, including a full muster. The scenario will not be disclosed in advance. Muster wardens will attend a refresher on 7 October. Visitors and contractors on the day will be briefed at induction.
